Insurance
Lifetime Insight accepts most major commercial insurance plans, including:
- BCBS
- Cigna
- Aetna
Coverage is subject to verification of benefits. Patients are responsible for any applicable copays, coinsurance, or deductibles at the time of service. Please notify our office immediately if your insurance changes. If your insurance denies coverage due to your deductible, coinsurance, or out-of-network status, you will be responsible for the self-pay rate.
No Surprises Act
Federal Protections Against Surprise Billing
Lifetime Insight participates with BCBS, Cigna, and Aetna insurance plans. Patients with these commercial insurance plans are protected by federal law (the No Surprises Act) from surprise bills and balance billing.
What this means for you:
- You pay only your in-network cost-sharing amount (copay, coinsurance, or deductible as specified in your plan)
- Lifetime Insight cannot bill you for any amount beyond what your insurance plan would pay an in-network provider
- You are never required to give up these protections

Self-Pay & Good Faith Estimate
In accordance with federal law, the following self-pay rates apply to patients without insurance or with out-of-network coverage:
- Initial Psychiatric Evaluation (55 min): $495
- Psychotherapy Visit (55 min): $495
- Extended Visit (45 min): $425
- Extended Medication Follow-Up (up to 25 min): $320
- Brief Medication Follow-Up (up to 15 min): $235
Payment is due in full at the time of service. A valid credit card is required on file at all times.
Superbills are available upon request for patients seeking out-of-network reimbursement from their insurance carrier. You must file for reimbursement, we are not able to do this for you.
We accept all major credit cards and ACH. If you need a payment plan, we are able to issue you a PayPal invoice and funding for no-interest payments is done directly through PayPal.
Certain services are not covered by insurance and are billed directly at a pro-rated rate of $495/hour. These include:
- Letters for schools, housing, or employers
- Disability paperwork
- Forms or reports outside of scheduled visits
- Phone calls exceeding 5 minutes
Disability paperwork is considered only after an established treatment relationship and at physician discretion. This practice does not provide documentation for Emotional Support Animals (ESA) or California state disability claims.
We require at least 24 business hours notice for cancellations or rescheduling. Fees are automatically charged to the card on file:
- 45–55 min visits: $425
- 25 min visits: $250
- 15 min visits: $175
Arrivals more than 5–10 minutes late may be considered a no-show depending on visit length.
